Meredith is back from visiting Derek in Washington, D.C., but her sister, Dr. Pierce, is being awfully suspicious of her trip. Pierce was the one to watch Meredith’s kids while she was away, and apparently she knows something we don’t. Uh oh.

Apparently, Meredith is not the only one who has had an interesting weekend. When we get to Catherine and Dr. Weber, they are kissing, and she can’t keep her hands off of him. We find out that they spent the weekend together, so they are back on!

Somewhere out in the woods of Washington state, Bailey is helping her husband and his brother scatter their father’s ashes in the woods. It was their father’s wish, and it’s sweet that they are doing it together despite Bailey’s paranoia about it being illegal.

Unfortunately, not everything goes smoothly. When Ben and his brother decide to “race” each other to the top of a hill, suddenly his brother doesn’t feel well and BOOM. He falls right down the hill, rolling past Bailey and just barely missing the pile of stones they laid down.

As the episode continues, everyone keeps asking Meredith about her weekend with Derek, but she remains vague. It turns out that Ben’s brother, Curtis, is fine with the exception of some tenderness near his liver. When Bailey and Ben demand they run more tests on him, Curtis freaks out and refuses. What’s up with that?

It turns out that his liver enzymes are off the chart and the rest of his lab results are not good. On top of that, his estrogen levels are too high for a young man his age, says Grey. They find out he needs his spleen removed, and Ben berates Curtis with the assumption he’s been doing drugs behind everyone’s back. Curtis assures him that’s not the case, so Ben storms out and demands more testing be done.

However, Bailey has an idea what could be going on — and she thinks he’s becoming a transgender woman. When she asks Grey about it, Grey refuses to answer and tells her she needs to ask Curtis. Whoa!

Meanwhile, Dr. Herman sexually propositions Dr. Hunt. Yes, really. It all goes down when he finds out that Herman — first name Nicole — moved a couch out of the attendees lounge into another one. Then, she offers him a chance to “try it out” with her. He finds a way to politely decline, but she tells him she’s just joking anyway. I don’t buy it!

Later on, Hunt runs into Amelia and tells her that Herman has been flirting with him. She immediately seems taken back by this, and it results in an awkward conversation about dating co-workers that Hunt abruptly walks away from. I spy a new couple on the horizon!

Meredith later runs into Dr. Pierce again and she finally reveals the truth: Derek called her three times while she was away asking where he was. But wait, wasn’t Meredith supposed to be with Derek? This is awkward.

“I want to be there for you, but I like Derek. I hate lying,” Pierce tells Grey, and her honesty is refreshing. Unfortunately, Meredith doesn’t get a chance to respond because Curtis starts coughing up thick, dark blood out of nowhere. Gross.

When Ben finds out what happened he goes running to Bailey and he’s convinced, again, that his brother must be using some sort of street drug that tore up his insides. Bailey talks him down and tells him that Curtis isn’t using drugs, but that he will need to talk to Curtis himself to find out the truth.

Ben immediately flips out and he’s furious that Miranda won’t tell him what’s going on, so she caves. And yes, it’s true, Curtis is transitioning. “Your brother is becoming your sister,” Bailey tells Ben, who remains stunned for a moment before bursting into laughter.

At first he doesn’t believe her and he even asks if Curtis “put her up” to what he thinks is a joke, but it’s not — and once it hits him he is stunned again.

Bailey then goes to check on Curtis in surgery and everything is going well, but when she returns to tell Ben he is clearly in denial about the transition. He tries to tell Bailey that Curtis is just upset about their dad dying, that he’s no serious, but Bailey has to talk him down again. Curtis is, in fact, serious, and he wants to live life as a woman.

In other news, Catherine somehow convinced Webber to let her scrub into a surgery. As soon as she was in the surgery, she finds an excuse to page her son, Jackson, and when he arrives she starts questioning him about April. How is she? How is he? How are they doing?

It’s no surprise Jackson is annoyed by this, but he does confess that things are tense at home and nothing he does seems to help April. In return, Catherine offers him some advice: back off. Not too far, just far enough that April can have some space and he will still be there when she needs him. It’s sweet, but he can’t see past being annoyed so he storms out of the O.R.

After stitching up Curtis, Pierce and Meredith get into it outside of the operating room. Pierce doesn’t want to be invasive, but she’s really upset with Meredith for “cheating on Derek. If that is what she was doing when she disappeared for the weekend.

No matter how upset she is, Meredith refuses to reveal what she was doing. I can’t help but wonder if she went to see Cristina. It would make sense, wouldn’t it? She’s been so stressed out about Derek, and Alex isn’t being a good “person,” and she needs Cristina.

Once Curtis is awake after his surgery, Ben goes into his room to talk to him about his transition. He doesn’t understand, he thinks it’s not real, and he also doesn’t believe Curtis when he says he’s known that he wanted to be a woman since he was five.

It’s heartbreaking to watch Ben berate his little brother about wanting to transition into a woman, but it’s the cold hard truth about how so many people are still unable to accept these things. Bailey catches the end of their argument and tries to stop Ben from storming out, but Curtis tells her not to bother. How sad.

She chases after him anyway and tells him how disappointed she is in him. Ben is angry, but he reveals that it’s not that he can’t accept that Curtis wants to be a woman. It’s that Curtis didn’t tell him. He feels betrayed that Curtis didn’t open up to him, and, even worse, he feels as if Curtis lied to him
